Working in collaboration with families and the individual with disabilities, a Planning Journal and Letter of Intent are created by recording personal information, instructions, family history, and other information so that future caregivers (if applicable) can make informed and timely decisions and provide independent individuals with resources and assistance to support their needs and decisions.

If needed, this includes developing a comprehensive budget to identify needed financial commitments. To provide current and future supports, Life Services gets to know people through the development of the Planning Journal and Letter of Intent. This knowledge (and then the Keeping Connected Program), flows into the Advocacy Program – identifying what is needed now and in the future to support and secure the highest standard of life possible.
